We have received your Client Intake Form and we will be in contact with you shortly to receive payment and to schedule an appointment. You may also be asked to fill out additional intake forms as a part of the onboarding process.
CC TeleHealth will reach out to you within 1-2 business days via phone to receive your credit card information for the $100 down payment. The balance will be processed after your initial appointment.

NOTE: You will be responsible for the additional cost of the prescribed medications and nutraceuticals which will be order by us through one of our partner pharmacies.
